SUPPLY DROP-OFF /  MEET-THE-TEACHER DAY

Tues., Aug 16th – 8:30 to 10:30am – PreK to 8th Grade
DOORS TO ENTER:

• Door 1 – Main Office (observe parking signs on Flagstaff Ln)
• Door 12 – Early-Childhood (parking lot at the corner of Flagstaff Ln & Grand Canyon)
• Door 21 – Library (Located off the playground/center parking lot)
• Door 38 – Jr. High Hallway (is the drop-off / pick-up door)

SUPPLIES—Bring ALL supplies & be sure your child’s name is on all items:

• Student names should be written (in Sharpie) on the tags of any sweatshirts or sweaters to ensure proper return if lost.

USED UNIFORM SALE—Visit the lunchroom (cash or checks--payable to St. Hubert FSA)
EXTENDED CARE SIGN-UP/QUESTIONS—Visit the school office (Click here for the Extended Care Letter/Registration Form) 
VOLUNTEER OPPORTUNITIES—Visit the marketing table in the lobby to register using a QR Code
PAPERWORK TO TURN IN THE CLASSROOM:

• INFORMATION FORM — (Click here for the Information Form to complete) 
• HEALTH FORMS—Physicals, dental & eye exams—please drop off in the classroom
  • If medication is needed at School--Visit the school office to drop off forms & medication
• T-SHIRT SIZE NEEDED—The teacher will have a sign-up sheet for you to select the size.
• BUS PASS--Will be in an envelope on your child’s desk. Please review ASAP for bus information & attach to their backpack.
• FREE & REDUCED LUNCH FORM—This form will be in an envelope on your child's desk. Complete & return on the first day of school

GYM DAY -- gray t-shirt & blue gym shorts/sweatpants are to be worn to school

• PreK - 3rd Grade -- Mondays  (PreK students do not have gym uniforms)
• 4th - 8th Grade -- Tuesdays
